Everyone Health will be working in collaboration with Greenwich Leisure Ltd (GLL) and Dragonfly Leisure Trust (DLT) to offer all referrals for Exercise Referral services a 1-2-1 behaviour change consultation prior to starting a programme. This service will require a Health Care Professional referral before we can make the onward referral to the leisure centres.
This service is recommended for people over 16 years with long-term health conditions to move more to improve their health and wellbeing.
Who is Eligible for the Service?
All low and medium risk conditions in accordance with the Irwin and Morgan risk stratification tool. For example;
- Hypertension (>160/100)
- Diabetes Type 1 & 2
- Cardiac rehabilitation patient (with a qualified instructor)
- Cancer rehabilitation patient (with a qualified instructor)
- Cancer prehabilitation (with a qualified instructor)
- Overweight/obesity (BMI>27)
- Hyperlipidaemia (> 5mmol/l)
- Asthma/COPD
- Depression, anxiety or mental illness
- Neurological conditions (e.g. Multiple Sclerosis)
- Mild musculoskeletal conditions (arthritis, osteoporosis, simple low back mechanical pain) where physical activity will provide symptomatic relief
- Antenatal/Postnatal
- Physical disabilities where independent physical activity is suitable
- Long Covid
Exclusion Criteria
- Angina/Heart Failure/Arrhythmias/Post MI
- Hypertension (> 180/100)
- Recent stroke (<3 months)
- Any medical condition that is not controlled, stable or is exacerbated by exercise
- Any musculoskeletal conditions that do not allow independent exercise
- Falls or at risk of falls
- Pre-natal up to 28 weeks and up to 3 months post-natal
- Any other condition that restricts ability to exercise safely

What does the Programme involve?
For a 12 week period, clients will be encouraged to build up their weekly activity levels through attending sessions at the centre of their choice either with GLL or DragonFly Leisure Centres.
All clients will take part in an initial assessment at the beginning and a completion assessment end of their 12-week programme. Once they have completed the programme, clients will be encouraged to independently continue with regular physical activity and will be provided with information on local activity opportunities.
Who can make an exercise referral?
- GPs
- Practice Nurses
- Specialist Nurses e.g. Health Check Hub, Cardiac Nurses
- Specialist Cardiac Physiotherapists
- Referrals can be initiated by a mental health professional by telephone or letter to the patient’s GP whose authority is required, and who can then send a referral to the programme.
